Adults with Chronic Conditions

We help provide resources and the support needed for a journey to better health. In the United States, chronic diseases like heart disease, cancer, and diabetes are responsible for most deaths and drive the vast majority of healthcare spending making up roughly 90% of the nation’s $4.9 trillion in annual health care costs (CDC 2025).

Why Enroll?

Our Community Health Workers (CHWs) are from Stark County and a vital resource in creating beneficial outcomes. We serve as a bridge between healthcare systems and the communities they serve, helping individuals manage chronic conditions and improve overall well-being. We want to help you succeed and meet you where you are.

How We Impact

  • Address social determinants of health (SDOH)

  • Support economic, social, and environmental factors that shape access to care and health outcomes

  • Meet monthly to learn what is most important  and share resources

  • Follow up between visits to ensure the appropriate services and needed care

  • Build trust with our local families as trusted community partners

Our Outcomes

CHWs help remove the barriers that often prevent people from getting care. These barriers include financial challenges, transportation issues, language or cultural differences, and limited health literacy.

CHWs guide individuals through complex healthcare systems, help them understand their diagnoses, and assist with scheduling and attending appointments, ensuring that more people get the care they need to live healthier lives.
