In the United States, chronic diseases like heart disease, cancer, and diabetes are responsible for most deaths and drive the vast majority of healthcare spending making up roughly 90% of the nation’s $4.9 trillion in annual health care costs (CDC 2025).

Our Outcomes
CHWs help remove the barriers that often prevent people from getting care. These barriers include financial challenges, transportation issues, language or cultural differences, and limited health literacy.
CHWs guide individuals through complex healthcare systems, help them understand their diagnoses, and assist with scheduling and attending appointments, ensuring that more people get the care they need to live healthier lives.
